Click on the link below to fill out and submit the Registration form. You will need to have Adobe Acrobat Reader 9 installed. We recommend that you print a copy for your records. You must fill out one registration for each student.
If you use a webmail system (for example AOL, Yahoo) you may be asked to save the document on your computer, then email it to us as an attachment. Alternatively, you may PRINT your filled out form and MAIL or FAX it to us. Please don't forget to make your payment online below and type your confirmation # where required on the registration form.
